The Transition to Adulthood team at YFS supports young people who are preparing to leave care, transitioning from out of home care, or navigating adulthood after their care experience. The Extended Post Care Support program forms part of this growing team, creating exciting opportunities for passionate individuals who want to make a lasting difference. This team focuses on ensuring young people leave care with the skills, resources, relationships and support they need to achieve positive long-term outcomes and build the future they want for themselves.
The Extended Post Care Support program is an exciting Queensland Government initiative that recognises successful transitions to adulthood take time. The program provides intensive, holistic support for young people up to the age of 21, alongside significant financial assistance for young people living independently, creating opportunities for young people to pursue their goals, strengthen their independence and thrive well into adulthood.
Our team works alongside young people, carers and service providers to build capability, create opportunities and support young people to achieve their goals. We are looking for someone who brings energy, initiative and a genuine belief in the potential of young people, and who can think creatively when challenges arise. This is an outreach role, working in community and meeting young people where they're at. Young people leaving care need support with the basics like establishing payments and finding appropriate housing, accessing mental health and allied health services, engaging with training and employment opportunities, just to name a few.

YFS is a leader in Youth Services in the Logan, Redlands and surrounding areas, and our youth programs are expanding. Our integrated approach is designed to deliver better outcomes for our clients. In this role, you will work with other teams across our range of youth programs, housing specialists, young parents’ programs and legal services.
Role location: Slacks Creek
Role type: 2 x Maximum term - full-time roles available until 30/6/2027
Pay range: $105,909 a year with salary packaging exempt from Fringe Benefits Tax available along with a monthly accrued day off.
You will need to have a minimum of three years’ experience working with young people, and an understanding of the challenges and barriers that young people in out of home care face as they transition to adulthood. Experience working in the child protection sector would be a great bonus.
Knowledge of the social and community housing and private rental sector would be favourable, or a willingness to learn and develop skills and expertise in this area will be critical.
You will be proactive, adaptable and solutions-focused in a dynamic environment. You will enjoy being part of a team that values creativity, innovation and finding practical ways to help young people overcome barriers and move towards their goals.
You will have the ability to work independently, recognise and respond to the barriers that young people with a care experience may face, and provide practical, purposeful support that helps young people build momentum towards their goals. You will approach your work with optimism, persistence and a commitment to achieving meaningful long-term outcomes.
We are looking for someone who is able to engage with young people from a range of backgrounds including a history of complex trauma. You will be able to build productive working relationships with a network of other professionals and stakeholders – you know that you can’t do it alone and value the input of others.
