We are seeking an experienced and dedicated Wound Care LPN to join our clinical team. This is an excellent opportunity to specialize in wound care while making a meaningful impact on residents’ health and recovery.

As a Wound Care Nurse (LPN), you will be responsible for providing specialized skin and wound care to residents under the direction of the physician, Director of Nursing, and/or Medical Director. You will play a key role in promoting healing, preventing complications, and improving overall resident outcomes.

Key Responsibilities

  • Provide direct wound care and skin treatments in accordance with physician orders
  • Assess, monitor, and document wound progress and treatment effectiveness
  • Assist in developing and modifying individualized care plans
  • Collaborate with the interdisciplinary team to ensure optimal outcomes
  • Educate staff on proper wound care techniques and prevention strategies
  • Maintain accurate and compliant clinical documentation
  • Ensure adherence to all regulatory, safety, and infection control standards

Qualifications

  • Active, unencumbered LPN license in South Carolina
  • Graduate of an accredited Practical Nursing program
  • Current CPR certification required
  • Ability to work 12-hour shifts
  • Experience with PointClickCare (PCC) preferred
  • Prior wound care experience strongly preferred
  • Strong clinical judgment and attention to detail
  • Passion for delivering compassionate, resident-centered care
