QualificationsAbout the Role
As a CMA Coordinator (Service Coordinator/ Case Manager), you will play a key role in supporting individuals and families by coordinating services and ensuring high-quality, person-centered care. This position works within a dynamic, highly regulated environment and focuses on building meaningful relationships while helping individuals access the services they need.
You’ll be working both in the community and in office/virtual settings, making a direct impact in the lives of people served in and around Woodland Park and Teller County.
What You’ll Do
- Coordinate and develop individualized service plans to support the needs of the people we serve
- Monitor services and ensure compliance with state and federal regulations
- Build relationships with individuals, families, and community providers
- Advocate for person-centered care and support individuals in achieving their goals
- Stay informed on Medicaid Waiver programs and Long Term Services and Supports (LTSS)
- Utilize technology (Microsoft 365, databases, virtual platforms) to document and manage services
- Conduct in-person visits in homes, community locations, and care settings
- Attend meetings, trainings, and collaborate with internal teams
- Maintain accurate records and ensure timely completion of all documentation
- Travel within the community (Woodland Park and surrounding areas) as needed
What We’re Looking For
- Bachelor’s degree in human services, social work, psychology, or related field OR equivalent experience working with LTSS populations
- Strong organizational and time management skills
- Excellent communication and relationship-building abilities
- Ability to problem solve, prioritize, and work independently
- Comfort working with technology and maintaining detailed records
- Empathy and a passion for supporting individuals with disabilities and/or long-term care needs
- Valid Colorado driver’s license and reliable transportation
Preferred Experience
- Experience with Medicaid Waiver programs
- Background working with individuals with disabilities
- Knowledge of community resources and service coordination

The Resource Exchange (TRE), serves over 12,000 infants, children, teenagers, adults, and seniors in El Paso, Pueblo, Teller, and Park counties. Advocating for independence and inclusion, TRE partners with children and adults who have a variety of disabilities, delays, mental health or long-term care needs. We do this using a person-centered approach in coordinating care, promoting choices, and collaborating with community partners.

TRE has relationships with over 200 community partners to which we refer the people and families we serve to increase opportunities, promote choice, and enhance life in the community. TRE is driven by the core values of respect, integrity, partnership, excellence, and accountability.
