WMO Consultant
Location: Middelburg, Netherlands
About the Role
Our client is seeking an experienced WMO Consultant to provide support to residents requiring assistance with independent living and social participation. You will be responsible for managing the entire support process, from the initial assessment through implementation and evaluation, ensuring residents receive appropriate services and provisions based on their individual needs.
Working as part of a multidisciplinary access team, you will collaborate with colleagues from various social support disciplines to deliver effective, person-centred solutions.
Key Responsibilities
* Conduct intake interviews with residents by telephone and through home visits to assess support needs.
* Analyse individual situations and determine appropriate support and care solutions.
* Advise residents on available services, community resources, and customized support options.
* Assess applications for household assistance, mobility aids, transportation services, home adaptations, guidance, and day-care support.
* Identify vulnerable or high-risk situations and take appropriate action.
* Coordinate, implement, and monitor support services and provided facilities.
* Prepare clear reports, case documentation, and recommendations.
* Evaluate support plans and adjust services where necessary.
* Collaborate closely with multidisciplinary colleagues to ensure integrated support for residents.
* Ensure all assessments and recommendations comply with relevant legislation and policies.
Required Qualifications
* Bachelor's degree (HBO) or equivalent in Social Work, Social Services, Healthcare, or a related field.
* Several years of experience in a similar WMO Consultant or social support role.
* Up-to-date knowledge of the Dutch Social Support Act (WMO) and relevant legislation.
* Strong interviewing, assessment, and case management skills.
* Excellent written communication and reporting abilities.
* Ability to analyse complex situations and make well-founded decisions.
Preferred Experience
* Experience conducting home visits and client assessments.
* Experience with household support assessments and mobility or accessibility provisions.
* Experience with guidance and day-care support services.
* Experience working within multidisciplinary social care teams.
* Knowledge of local government or public sector social support services.
