The Case Manager oversees a comprehensive program of services, including individual and group assessments, counseling, and linkage to health, educational, entitlement, and vocational resources to support successful community integration. This role ensures the delivery of all casework services in alignment with ACT (Assertive Community Treatment) standards. The position may involve supervised or unsupervised interaction with consumers in residences, day treatment settings, and habilitation programs.
Essential Responsibilities
Conduct screenings and assessments to identify consumer needs.
Coordinate case management services and develop individualized service plans.
Assist consumers with obtaining and maintaining entitlements and benefits.
Serve as a clinical liaison with internal and external providers.
Ensure service quality through regular supervision, case conferences, and consultation.
Develop and implement casework policies and procedures.
Collaborate with ACT team members to create treatment plans and coordinate care.
Facilitate consumer groups and provide counseling and crisis intervention as needed.
Collect, analyze, and report client and program data for evaluation and planning.
Ensure documentation is completed timely and in compliance with agency and OMH regulations.
Maintain physical site compliance with agency and OMH standards.
Participate in OMH-approved ACT orientations and required trainings.
Assist consumers with transportation, appointments, and accessing community resources.
Support skill-building activities in areas such as budgeting, grooming, nutrition, transportation, housekeeping, community use, vocational development, and social skills.
Act as a positive role model and support consumers in follow-up care and resource navigation.
Enter all service data and visit notes into the VelloHealth system.
Represent the program at meetings and trainings as assigned.
Maintain confidentiality and adhere to incident-reporting procedures within 24 hours.
Perform additional duties as assigned and assist other staff during absences or shortages.

Ability to maintain accurate written documentation and communicate effectively with staff and supervisors.
Flexibility to work varying schedules and locations.
Ability to operate an agency vehicle.
