Under the leadership of the Wellness Lead, the Wellness Partner delivers engaging, inclusive and person‑centred wellness activities that support residents’ physical, emotional,socialand cognitive wellbeing. The role works directly with residents to understand their individual interests, culturalbackgroundsand abilities, enabling meaningful participation that promotes independence, choice,dignityand quality of life in alignment with organisational expectations and the Strengthened Aged Care Quality Standards.
In this role, you will:
Facilitate inclusive group and 1:1 wellnessactivitiesthat support residents’ physical, emotional,socialand cognitive wellbeing
Build respectful relationships with residents, encouraging independence, dignity, choice and participation through meaningful engagement,companionshipand emotional support
Collaborate with care staff, clinical teams, families, volunteers, the Wellness Lead and Home Services Lead to ensure programs align with resident needs
Guide,supportand engage Volunteers so they feel confident,informedand able to contribute effectively to resident wellbeing
Seekresident and family input to improve programs, contribute ideas in meetings, and help embed HELF Services into everyday home operations
Complete lifestyle assessments (with supervision),maintainaccuraterecords, andobserveand report changes in resident wellbeing in line with escalation processes
Prepare,organiseandmaintainactivity spaces,materialsand equipment to ensure safe, welcoming andhazard‑freeenvironments
Followorganisationalpolicies (manual handling, WHS, IPC, emergency procedures) and contribute to continuous improvement by sharing feedback and observations
What makes a difference for us?
Certificate IV in Leisure and Health or an equivalent qualification
Experience delivering group and 1:1 activity in aged care or community settings
Practical experience in creative, social,cognitiveand physical activity facilitation
Understanding of dementia care, person‑centred practice and cultural safety
Knowledge of infection control, manual handlingpracticesand Strengthened Aged Care Quality Standards
Exposure to person‑centred care principles and a sound understanding of the needs of older adults
Experience working as part of a multidisciplinary team, including nurses, allied health professionals, carestaffand volunteers
Strong ability to build positive,respectfuland trusting relationships with residents and their families
Ability to design and adapt lifestyle and wellness activities for residents with cognitive or physical limitations
Understanding of dignity, culturalsafetyand individual choice in care delivery
Satisfy the relevant probity checks required by legislation or organisational policy
