Transition Specialist – Full-Time | Vocational Services | Brick, NJ

Schedule: Full-time | Monday–Friday, 8:00 AM–4:00 PM (includes daytime programming and community-based activities)

Department: Vocational Services

Compensation: $21.50/hour | Hourly position

As an Employment Transition Specialist, you will help adults with intellectual and developmental disabilities strengthen workplace readiness, develop practical life skills, and gain valuable experiences that support long-term success in the community.

This position is ideal for someone who enjoys mentoring others, facilitating skill development, and working directly within community settings to help individuals build confidence and achieve personal goals.

What You’ll Do

As an Employment Transition Specialist, you’ll help individuals participate in structured programming designed to build independence, strengthen vocational skills, and encourage active community involvement.

Skill Building & Individualized Support

  • Teach practical life and employment-related skills, including communication, problem-solving, social interaction, and daily living tasks
  • Facilitate structured activities and learning experiences that promote job readiness and personal development
  • Adapt instruction and support methods based on each individual’s strengths, preferences, and goals
  • Encourage independence, self-advocacy, positive decision-making, and confidence-building

Community-Based Learning & Employment Exposure

  • Support individuals at community-based learning sites, including volunteer opportunities, job exploration experiences, and inclusive settings
  • Develop and maintain positive working relationships with local employers and community organizations
  • Safely transport participants to and from community locations using agency vehicles

Documentation, Planning & Goal Tracking

  • Maintain familiarity with each individual’s Individual Service Plan (ISP) and related objectives
  • Participate in ISP meetings and provide feedback regarding progress and developmental needs
  • Assist individuals in working toward vocational and personal milestones
  • Complete required documentation, including progress records, medication tracking, incident reports, and daily notes
  • Implement behavioral support strategies and appropriately respond to behavioral or crisis-related situations when necessary

Health, Safety & Program Standards

  • Administer medications according to agency procedures and guidelines
  • Follow individualized plans related to health, dietary needs, adaptive equipment, and behavioral supports
  • Promote a safe, respectful environment that protects the dignity, privacy, and well-being of all individuals served

Requirements

What We’re Looking For

  • Bachelor’s degree in Human Services or a related field required
  • Valid New Jersey driver’s license in good standing ( 5 points or fewer)
  • Strong written and verbal communication skills
  • Ability to work independently, solve problems, and collaborate effectively with colleagues
  • Comfortable building relationships with community organizations and employment partners
  • Ability to meet the physical demands of the role, including lifting 25+ pounds, assisting with mobility needs, and traveling throughout community settings
