Vocational Nurse II - Epidemiology-(2605365)

The Licensed Vocational Nurse II (LVN II) is responsible for providing high-quality nursing care to patients with predictable healthcare needs, functioning as an integral part of the clinical team. This role requires advanced proficiency in primary and/or specialty areas, ensuring comprehensive patient care while adhering to established standards of practice for vocational nurses. The LVN II contributes significantly to the overall efficiency and effectiveness of the clinical environment under the supervision of registered nurses, advanced practice nurses, physician assistants, physicians.

ESSENTIAL JOB FUNCTIONS

  • Clinical Nursing Responsibilities
  • Provide professional nursing care to patients with predictable healthcare needs.
  • Perform targeted nursing assessments and appropriate clinical documentation.
  • Monitor patient health status and communicate findings to providers and interdisciplinary team members.
  • Provide education regarding HIV disease management, medication adherence, preventive health measures, and self-care practices.
  • Ensure patient care activities comply with applicable UTMB policies, nursing standards, and program requirements.
  • Maintain readiness of clinical environments, supplies, and equipment necessary for patient care delivery.

Case Management Responsibilities

  • Conduct initial and ongoing assessments of patients' medical, psychosocial, financial, and support service needs.
  • Develop, implement, monitor, and update individualized care plans.
  • Coordinate care among healthcare providers, pharmacies, behavioral health services, and community-based organizations.
  • Facilitate referrals and linkage to Ryan White funded and non-funded support services.
  • Monitor patient engagement in care and identify barriers that may impact treatment adherence or retention in care.
  • Assist patients in accessing available community resources and support programs.
  • Provide advocacy to promote access to quality healthcare and supportive services.

Ryan White Program Responsibilities

  • Ensure compliance with Ryan White HIV/AIDS Program standards and documentation requirements.
  • Monitor care plan goals and outcomes according to program requirements.
  • Participate in quality management and quality improvement activities.
  • Assist with collection, tracking, and reporting of program performance measures and clinical outcomes.
  • Support audit readiness initiatives and corrective action plan implementation.
  • Maintain accurate and timely documentation within the electronic health record and required reporting systems.

Collaboration and Communication

  • Collaborate with physicians, advanced practice providers, social workers, pharmacists, data coordinators, and community partners.
  • Participate in interdisciplinary case conferences and team meetings.
  • Communicate effectively with patients, families, providers, and external agencies.
  • Promote a patient-centered approach to care coordination and service delivery.

Marginal or Periodic Functions:

  • Adheres to internal controls and reporting structure.
  • Performs related duties as required.

KNOWLEDGE/SKILLS/ABILITIES

  • Perform focused assessments and assist in developing care plans for assigned patient populations
  • Administer treatments as ordered.
  • Utilize EHR systems efficiently.
  • Communicate effectively with patients, colleagues, and supervising staff/faculty.
  • Provide clear and concise patient instructions.
  • Demonstrate competencies as outlined in competency based objective tool.
  • Assist with creating and implementing staff education, training, and development.

EDUCATION & EXPERIENCE

Minimum Qualifications:

  • Certificate or Associate of Applied Science (AAS)
  • 5 years of experience

Preferred Qualifications:

  • Bilingual in Spanish

LICENSES, REGISTRATIONS OR CERTIFICATIONS

Required:

  • Licensed vocational/practical nurse license issued by the Texas Board of Nursing or compact licensure.
  • IV therapy or other advanced training certificates in specialty care area.
  • Current Basic Life Support certification .

SALARY

Commensurate with experience.
