About the Role

We are seeking an experienced Virtual Nurse (RN) to provide remote clinical oversight and supervision for Support at Home care partners. This role plays a critical part in ensuring care delivery remains safe, compliant, and responsive through comprehensive clinical file reviews, documentation audits, and proactive monitoring.

Working virtually, you will identify changes in a consumer’s functional status, health needs, and risk profile, enabling timely escalation, reassessment, and appropriate referrals in line with Support at Home guidelines and funding requirements.

Key Responsibilities

Clinical File Review & Oversight

  • Conduct comprehensive virtual reviews of consumer care files, clinical notes, assessments, and service documentation
  • Identify changes in consumer function, health status, cognition, mobility, continence, skin integrity, nutrition, and psychosocial needs
  • Ensure care plans, progress notes, and assessments are current, clinically appropriate, and aligned with Support at Home requirements
  • Review incident reports, clinical risks, and monitoring data to identify emerging concerns or trends
  • Ensure documentation reflects evidence-based practice and person-centred care

Virtual Supervision & Clinical Support

  • Provide virtual clinical supervision and guidance to care partners and frontline staff as required
  • Respond to clinical queries related to documentation standards, care planning, and escalation pathways
  • Promote consistent clinical practice across providers and service settings
  • Support best-practice care delivery and early intervention

Reporting & Collaboration

  • Document outcomes of file reviews, identified risks, and recommended actions clearly and accurately
  • Provide structured clinical reports and summaries to internal leadership as required
  • Collaborate with nursing, allied health, intake, and quality teams to ensure coordinated follow-up
  • Contribute to quality improvement activities and Support at Home audits

Other duties as delegated to support the ongoing functioning of InOne Healthcare

About You

You are a clinically confident and detail-oriented Nurse who is:

  • Currently registered with AHPRA as an Registered Nurse
  • Experienced in aged care, community care, or Support at Home environments
  • Highly skilled in clinical documentation review and risk identification
  • Confident working autonomously in a virtual or remote setting
  • Knowledgeable in escalation pathways, reassessment triggers, and multidisciplinary care
  • Analytical, organised, and proactive in identifying clinical concerns
  • Committed to person-centred, compliant, and evidence-based care
