The Virtual Care Nurse works closely with a varied and efficient patient care delivery team. This role serves as a key resource in providing remote patient care. The main responsibility is coordinating timely, safe, and effective care using strong technical, organizational, and critical thinking skills.
The Virtual Care Nurse will coordinate all patient care activities remotely. These include screening, triage, and managing a multi-disciplinary team to ensure safe, timely, and quality care. This role demands a skilled and experienced clinician with comprehensive nursing knowledge. The nurse must also work closely with customers, physicians, and leaders, needing advanced communication abilities.
Primary Duties & Responsibilities
Uses the resources and knowledge of the virtual care team and service provider network to develop collaborative workflows, policies, protocols, and guidelines. These support safe, reliable, high-quality, and evidence-based care, built on clinical protocols.
Assigns activities or tasks related to patient care while maintaining accountability for the outcome and responsibility for the delivery of nursing care
Collaborates with bedside staff, physicians, and interdisciplinary teams to coordinate patient care and support clinical decision making.
Facilitates components of the admission and discharge process, including patient intake, care coordination, and patient education.
Provides patient and family education to support understanding of care plans, treatments, medications, and discharge instructions.
