Specialty: Urology Type of Position: Permanent Shift/Work Schedule: Clinic and OR Monday - Friday Work Hours: 40 hours/week (7:30 AM - 4:30 PM) Call Schedule: 1 weekend a month, plus 1-2 weeknights per week. Doctor is on backup call with AP taking 1st call. Start Date: ASAP. Credentialing takes 120 days  Facility Type: Hospital based with clinic Clinical Setting: Inpatient, Outpatient, Telemedicine Patients Per Day: 20-30 Procedures required: General Urology. Robotics available . Support Staff: CMA/LPN/RN/APC Credentialing Time Frame: 120 days   Location: Stillwater MN, Hudson WI, New Richmond WI, Amery W I Additional Location Information: Multiple sites in MN/WI available. Physician would not work at more than two sites. Credentialing and Licensing Requirements: Need both MN and WI Licenses Board Certification in what specialty: American Board Certified or Eligible in Urology Willing to accept new grad or Board Eligible candidate: Yes Willing to wait for State Medical License: Yes Willing to accept J1 or H1 visa candidates: No Base Salary/Total compensation range: 2 year guaranteed minimum $400,000 base salary (negotiable)with production overage Bonus/RVU, sign on, relocation or Incentive structure: Sign-on, relocation, production Benefits Offered: Medical, Dental, Vision, Retirement, CME, Disability
