Job Description

St. Croix Health is seeking a full-time (1.0 FTE) team member to join our organization. This position is scheduled Monday- Friday 7:00am-3:30pm with no weekends.

The Referral Unit Coordinator is responsible for performing a variety of clerical functions to support the providers and patient care staff. This position enters patient calls/messages into the Electronic Medical Record database, ensure patient medical records and information that is needed is available and documented, coordinates internal and external referrals and schedules patient appointments. The Referral Unit Coordinator answers phone calls and relays messages to care team members and other resources necessary for patient care.

Essential Duties and Responsibilities:

Coordinates activities to support provider and patient care staff including monitoring provider schedules, is aware of add-ons, cancellations, no shows and overbooks, and schedules patient appointments for any testing, referral or follow up appointments that are needed.

  • Prioritizes referrals by urgency; Addresses in a timely manner
  • Sends referrals to external health care facilities for services not provided at St. Croix Health
  • Monitors internal/external referrals and coordinates with patient on next steps
  • Make referral appointments including scheduling preliminary appointments/testing
  • Demonstrates ability to provide care or service adjusting approaches to reflect developmental levels and cultural differences of population served

Performs telephone or direct communication with care team in a timely fashion to meet patient needs including requesting/obtaining patient reports as necessary.

  • Works with outside healthcare agencies to facilitate patient appointments and referrals
  • Follows up with patients requesting referral status
  • Works with other internal departments for care coordination
  • Peer and customer feedback
  • Timely relays patient information to the care team
  • Communicates with provider care teams to ensure patients’ needs are met
  • Follow up with patients to ensure that they are receiving the care that was recommended by their provider

Requirements

Education & Licensure:

  • High School diploma/GED

Knowledge, Skills & Abilities:

  • Excellent communication and listening skills
  • Demonstrated ability to work independently with little direction and manage multiple demands and priorities
  • Time management and prioritize multiple tasks

Physical Requirements

  • Prolonged periods of sitting at a desk and working on a computer
  • Prolonged periods of standing, bending, and reaching
  • Must be able to lift up to 50 pounds at times
