Responsibilities

• Support individuals in achieving personal and community-based goals
• Assist with daily routines and life skills development
• Promote safe, healthy, and independent living
• Provide transportation to appointments, activities, and community outings
• Monitor health, safety, and well-being
• Document services and progress accurately
• Work collaboratively with families and interdisciplinary teams

Qualifications: What We're Looking For:

• Caring and positive mindset
• At least 18 years of age
• Valid driver's license
• High school diploma or GED
• Ability to complete required training and certifications
