Job Description

The Triage Specialist conducts a thorough telephonic nursing triage for acute and chronic medical complaints and uses electronic health record (EHR) EPIC for documentation of patient interactions regarding triage protocols, appointment scheduling, homecare nursing instruction, and collaboration with other healthcare team members.

  • Monday-Friday - 8:00am - 6:00pm, 4-10 hour shifts variable days during the week and every other weekend
  • Westerville, OH

Essential Functions and Responsibilities:

  • Conduct telephone triage for patient health complaints using an electronic triage assessment tool.
  • Navigate multiple electronic platforms to assess current health complaint, patients’ electronic chart, PCP appointment scheduling, and ancillary center scheduling.
  • Document nursing triage notes into the EHR system for clear and concise documentation.
  • Document phone call details in tracking system for department data tracking and collection.
  • Assess, identify, and close patient care gaps.
  • Develop positive and effective working relationship with COPC staff, offices, and physicians in order to deliver high quality patient care.


Qualifications:

Education, Licensures & Certifications

  • Required: Bachelor’s degree in Nursing
  • Required: Active and unrestricted registered nurse license in the state of Ohio
  • Required: 2 years minimum of Register Nurse experience in a hospital and/ or medical practice setting
  • Preferred: Experience in telephonic nursing triage

  • Ability to offer flexible work hours, including the ability to work weekend hours
  • Basic knowledge of Microsoft Office Programs, such as Excel
  • Proficient knowledge of electronic health records
  • Excellent listening, communication and documentation skills
  • Proficient in clinical practice protocols and telemedicine patient care
  • Strong knowledge of medical terminology
  • Excellent relationship building skills
  • Possesses customer service-oriented mindset
