Overview

The LPN Triage Nurse Liaison serves as a critical coordinator of care, supporting the full continuum of patient management' from perioperative to acute and post-acute phases. This role functions as a key communication link between patients, families, healthcare providers, and support services to ensure timely, compassionate, and efficient care delivery. Serve as the primary liaison offering education, support, and guidance in an ambulatory care setting. Provide in-clinic support as requested by management. Cross-cover duties with other triage nurses to ensure continuity of care during staff absences. Advanced knowledge and clinical experience in specialty specific nursing or a related ambulatory care setting is preferred. Strong communication, documentation, and organizational skills. Ability to work collaboratively across interdisciplinary teams. Proficiency in patient care coordination, including scheduling and triage functions. Adheres to institutional policies and regulatory standards as directed by the Nurse Manager and Medical Director.

Qualifications

  • Graduate of an accredited Licensed Practical Nurse (LPN) program.
  • Current licensure as an LPN by the State of Florida.
  • At least 1 year of specialty-specific nursing experience or 2 years of clinical experience as an LPN required.
  • Strong initiative and ability to manage multiple responsibilities effectively.
  • Excellent problem-solving skills and the ability to work both independently and as part of a team.
  • Strong interpersonal and communication skills to support collaboration across healthcare disciplines.

Licensure/Certification/Registration

  • Current licensure by the State of Florida as an LPN.
  • Basic Life Support (BLS) certification required or obtained at the first opportunity with employment.

Motor Vehicle Operator Designation

  • Employees in this position will operate vehicles for an assigned business purpose as a non-frequent driver.
  • Frequent Driver Definition: An employee who uses their personal or Shands automobile:
  • At least once daily; or
  • At least five individual trips per week; or
  • Drives, on average, over 150 miles per week in the performance of their job.
  • The appropriate operator designation should be indicated on the Request for Personnel (RFP) form when the RFP is submitted to post the position.
