About You!

We are seeking an RN, Social Worker, OT, or Physiotherapist with Triage Delegate experience to join our multidisciplinary Health team working across metro Melbourne’s Southern & Eastern suburbs. This is a true casual role, ideal for someone happy to jump in to cover leave, assist during busy periods, or provide extra support whenever needed.

You will work within a primary health care framework to assess the needs of older persons and appropriate younger people with disabilities, within the region. Facilitate access to appropriate community and residential services, through a multi-disciplinary team approach.

Preferred candidates will have

  • Qualifications in a Healthcare background ie. Registered Nurse, Social worker, Physiotherapist or OT & registered with AHPRA or own relevant professional body where relevant.
  • Must have triage experience within the Single Assessment System
  • Ability to work independently and as a team.
  • High level of computer literacy skills
  • Ability to follow guidelines relevant to the position as well as demonstrated problem-solving capacity.
  • Current Australian driver’s license with own reliable, fully insured vehicle
  • Valid National Criminal History Check and Working with Children Check or willingness to obtain one.

What You’ll Do!

  • Conduct triage over the phone with the client or their representative to gather detailed information about their individual circumstances
  • Receive inbound calls using a 'no wrong door approach' to assist clients with their referral queries and navigating the aged care assessment process
  • Identify assessments based on urgency to assist with resource prioritisation
  • Assess whether the client requires a Home Support or Comprehensive Assessment.
  • Where appropriate, make recommendations for appropriately qualified assessment staff to conduct assessments based on the unique needs of the client (eg: cultural background, area of assessor specialty - homelessness)
  • Ensure all decisions are made in accordance with the Act, exercising due care, diligence, honesty, and good faith.
  • Verify the identity of the client or their representative and ensure all information is accurately documented.
  • Capture detailed information regarding the client’s needs, including any potential need for assistive technology, home modifications, or linking services.
  • Ensure the client has consented to the assessment process, including information sharing and representation by a third party, where applicable.
  • Provide clear and accurate documentation of the client’s needs, eligibility, and service recommendations.
