Overview
Social Worker, Medical Social Worker (Transitional Care) – Sitka, Alaska. This travel temporary assignment begins on 09/28/2026 and runs for 13 weeks. Compensation is $2,524-$2,554 per week with guaranteed 40 hours per week, on a 5x8 Days shift. The role operates in an inpatient, multidisciplinary care environment and centers on proactive, integrated social services care management to support patients and families from birth through end of life, focusing on discharge planning and placement needs.

Responsibilities
- Work with the inpatient care coordination team to assess biopsychosocial needs and support discharge planning, identifying barriers to discharge and coordinating community resources.
- Complete swing bed admission assessments and ensure patients and families understand participation requirements for subacute rehabilitation services.
- Facilitate or co-facilitate patient/family care conferences to explain illness, plan of care, and navigate the healthcare system.
- Assist patients with financial, housing, transportation, and DME needs; guide applications and referrals to state, federal, and community resources.
- Identify custodial, legal, or advance directive considerations and refer to appropriate specialists for crisis or behavioral health support; maintain accurate documentation in the medical record.

Requirements
- Education: BSW or MSW in social services required
- Experience: Minimum 2 years of hospital-based social work experience
- Licensure/Certifications: BLS preferred; active licensure not specified beyond degree
- Age: Must be at least 21 years old
- Vaccines/Background: Flu vaccine required; fingerprint cards required for ICPA background clearance (start date may shift if prints are not received within 1 week of signing)
- Skills: Strong assessment, discharge planning, and interprofessional communication; ability to work with diverse populations and maintain confidentiality
