Overview
Social Worker – Medical Social Worker in Lawrenceville, GA. This travel temporary assignment starts 06/08/2026 and runs 13 weeks with guaranteed 40 hours per week. Weekly pay ranges from $1,649 to $1,733. Shift is 12:00 PM to 8:30 PM, Monday through Friday, in an acute care setting. Note: No local candidates within 50 miles.

Responsibilities
- Conduct psychosocial assessments for admitted patients and participate in discharge planning.
- Assess social determinants of health and coordinate with the care team to address barriers to discharge and connect patients with community resources.
- Provide counseling to patients and families; advocate for patient preferences and rights.
- Develop, document, and maintain psychosocial assessments, care plans, and progress notes in accordance with policy and privacy regulations.
- Coordinate referrals for social work services, financial assistance, and post‑discharge supports such as housing or transportation.
- Collaborate with multidisciplinary teams to support timely patient flow and patient‑centered care.

Requirements
- Active GA LCSW license
- AHA BLS certification
- Minimum 2 years of recent medical social work experience in the specialty
- Acute/inpatient care setting experience
- Ability to work the listed shift and hours in Lawrenceville, GA
- Strong communication, assessment, and documentation skills
