MSW (Medical Social Worker) – Case Management in Everett, WA. Start date: 06/22/2026. This 13-week travel assignment is in a hospital setting with a Level II trauma center across two nearby campuses. Responsibilities
- Conduct biopsychosocial assessments for adult and geriatric patients and document plans of care.
- Lead discharge planning and care coordination, with referrals to home health, DME, rehabilitation, and long-term care as appropriate.
- Manage a typical caseload of 18–24 patients (up to ~30 on weekends), ensuring timely discharges and appropriate follow-up.
- Collaborate with interdisciplinary teams (PT, interpretation services, social services, pharmacy, transportation) to coordinate patient care.
- Document assessments, planning, and progress in EPIC; address guardianship and Medicare/Medicaid processes as needed.
- Support diverse clinical needs across Med-Surg, LLOS, and ED settings; participate in ongoing care coordination and patient-family education.
Requirements
- Active Washington state social work license in-hand (LICSW or LSWAIC accepted; non-clinical licenses LASW/LSWAA also acceptable).
- BLS certification.
- EPIC charting experience required.
- Minimum 1 year clinical experience in acute care, discharge planning, or care coordination; strong preference for diverse case management across Med-Surg, LLOS, and ED.
- Ability to float across hospital and sister campus (within ~2 miles); may involve cross-unit experiences including Colby and Pacific campuses if applicable.
- Weekend coverage: every other weekend required; holiday coverage as specified by compliance notes (up to 3 of 5 major holidays).
- Orientation: 40 hours prior to start.
- References: two references within the last 3 years required.
- Licensure notes: Washington license in-hand prior to start (pending licenses not accepted); COVID-19 vaccine not required.
