This travel‑assignment position supports after-hours hospice care across the Mesa County region, with care delivered in homes, assisted living and skilled facilities, hospitals, and a hospice care center. The role emphasizes symptom management, urgent visit responses, and end-of-life support for multiple service lines. The nurse will coordinate care from a home base between patient visits.

Responsibilities
- Provide after-hours hospice nursing care across diverse settings, including homes, assisted living, skilled facilities, hospitals, and a hospice care center
- Quickly assess patient and family needs; determine symptom management and urgent visit interventions
- Administer medications, monitor vitals, manage comfort measures, and respond to acute changes in patient condition
- Develop and modify care plans in collaboration with on-call teams; document findings clearly and promptly
- Communicate with patients, families, and interdisciplinary team members to ensure aligned, compassionate care
- Offer end-of-life support and symptom relief during after-hours encounters; maintain safety and infection control standards
- Work from home between patient visits and travel to patient locations as required
Requirements
- Current, active Registered Nurse license in Colorado
- Hospice or palliative care experience preferred; not required
- Ability to work 7:00 pm–7:00 am shifts with rotating nights
- Comfort delivering care across home, assisted living, skilled facilities, hospitals, and hospice settings; capable of coordinating care remotely between visits
- Willingness to travel within the Grand Junction/Mesa County area
