Responsibilities
- Deliver routine hospice nursing visits in outpatient settings and patient homes (approximately 4–5 visits per day)
- Assess patient condition, manage symptoms, administer medications per physician orders, and document care plans
- Collaborate with the interdisciplinary team to coordinate patient care and update electronic health records
- Travel between patient locations within a defined daily radius; maintain accurate visit timing and documentation
- Educate patients and caregivers on disease process, symptom management, safety, and transition planning

Requirements
- Active Massachusetts RN license required at time of submission
- BLS certification required
- Experience in hospice nursing required
- Ability to travel 30–50 miles per day; valid driver's license and reliable transportation
- Schedule requires Monday–Friday daytime hours (8:00 am–4:30 pm)
