Responsibilities

  • Assess home health and hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs; document findings and share evaluations with the care team
  • Develop, implement, document, and evaluate the Plan of Care to ensure quality and continuity of care
  • Collect data on physical, psychological, social, spiritual, and cultural factors influencing patient/family health; integrate data into the Plan of Care
  • Maintain up-to-date patient records, ensuring visits, problems, plans, actions, goals, and changes are accurately reflected
  • Apply infection control and OSHA standards during all patient encounters
  • Educate patients and families regarding care, medications, home safety, diet, and other relevant topics; use age-appropriate and culturally sensitive teaching methods
  • Supervise and provide clinical directions to HHA/STNA/LVN to ensure continuity and quality of service
  • Assess and reassess pain; implement appropriate pain management strategies and educate patient/family
  • Arrange referrals to appropriate agencies when needs exist
  • Monitor assigned cases for third-party payer compliance and coordinate on-call coverage as assigned
  • Communicate effectively with patients, families, team members, and other health professionals; participate in interdisciplinary meetings
  • Perform additional related duties as assigned

Requirements

  • Current state license as a Registered Nurse (Colorado) – Required
  • Education: ADN or Nursing Diploma program graduate – Required; BSN – Preferred
  • Experience: 1–2 years RN clinical care experience – Required; 2+ years in home health – Required
  • Certifications: OASIS experience – Required; CHPN – Preferred; CPR certification – Required
  • Software: Homecare Homebase software experience – Required
  • Other: Valid driver’s license and reliable transportation/insurance; ability to work in a home health/hospice setting
  • Additional: Must have a comprehensive clinical skill set including venipuncture, IV care, wound care, catheter/drain management, and appropriate use of durable medical equipment; strong communication and organizational abilities
