Responsibilities
- Provide a full range of professional nursing services to patients of all ages in the home setting, including admission, initial evaluation, and ongoing care planning
- Develop and adjust individualized care plans in collaboration with the patient, family, and interdisciplinary team
- Deliver direct patient care: monitor vitals, administer medications and treatments, manage accucheck and wound care, and respond to health changes
- Educate patients and families on disease management, safety, medications, symptom management, and care continuity
- Coordinate with home health professionals and paraprofessionals to ensure high-quality, coordinated care across the patient’s plan of care
- Document care provided in the charting system in a timely, accurate, and compliant manner and participate in care conferences as needed
Requirements
- Active Vermont RN license
- Minimum 1 year of prior clinical experience
- Ability to provide nursing care in patients’ homes and work within a home health/hospice context
- Strong communication and teamwork skills; ability to collaborate with an interdisciplinary team
- Willingness to work a daytime schedule (M-F) with potential weekend shifts
- Eligibility for a travel assignment and ability to meet assignment start and duration requirements
