Overview
Registered Nurse – Home Health (Travel) – Lancaster, PA. This 13-week travel assignment begins 06/01/2026, with 40 guaranteed hours per week on 8-hour shifts in a home health/hospice care setting. Travel placement through Healthforce Healthcare LLC.

Responsibilities
- Assess home health and hospice patients in stable and emergent situations; identify physical, psychosocial, and environmental needs and inform the Plan of Care (POC).
- Implement, document, and evaluate the POC to ensure quality and continuity of care; maintain up-to-date patient records reflecting visits, problems, actions, and goals.
- Obtain data on physical, psychological, social, spiritual, and cultural factors affecting health status; integrate data into the POC.
- Educate patients and families on care, medications, home safety, and self-care practices; use appropriate teaching methods and promote self-management.
- Supervise and provide clinical directions to HHA/STNA/LVN staff to ensure quality and continuity of service.
- Communicate effectively with patients, families, interdisciplinary team members, and referring providers; participate in interdisciplinary team meetings as appropriate; provide on-call services as assigned.

Requirements
- Current state license as a Registered Nurse (Pennsylvania) – Required
- CPR certification – Required
- CHPN certification – Preferred
- OASIS experience – Required
- Homecare/Homebase software experience – Required
- 1-2 years RN experience in a clinical care setting – Required
- 2+ years home health experience – Required
- ADN or Nursing Diploma program graduate – Required; BSN – Preferred
- Current driver's license; reliable transportation and insurance – Required
- Must be able to work in a home health/hospice setting; flexible schedule; travel locally
