Overview
Registered Nurse – Home Health travel assignment in Kaneohe, Hawaii. This temporary assignment begins 05/18/2026 for 13 weeks, with 40 guaranteed hours per week on a 5x8 day shift in a home healthcare setting. Weekly pay ranges from $2,366 to $2,445 per week.

Responsibilities
- Provide comprehensive patient care within a home health case management framework, including development, implementation, revision, and reporting of the case management plan.
- Serve as liaison among patient, family, nurse, physicians, and the multidisciplinary team to optimize outcomes; act as a consultant on specific patient items.
- Lead coordination of patient care across disciplines; monitor the appropriateness and timeliness of care.
- Ensure the interdisciplinary care plan aligns with the patient’s clinical course, continuing care needs, and covered services; monitor diagnostic testing, treatments, and procedures.
- Discuss with physicians the appropriateness of resource utilization, treatment plans, estimated length of stay, and discharge planning.
- Focus on complex patients, frequent emergency department utilizers, chronic pain and substance use concerns, and homeless patients; collaborate with case managers to ensure continuity of care; coordinate transfers to tertiary centers as needed.
- Provide oversight and collect data required for regulatory and accreditation compliance; support a focus study review of frequent ED visitors when indicated.

Requirements
- Hawaii registered nursing license (active and clear) required
- 2+ years Medicare-certified home health agency experience required
- Travel experience required
- Active and clear driver’s license and auto insurance; must have own transportation
- Proficiency with Homecare Home Base and OASIS required
- COVID-19 and influenza vaccines required (no vaccine declinations accepted)
- Education: Bachelor's degree preferred
- Experience: Case management experience preferred

- Mileage reimbursement at $0.43 per mile
- On-call requirement: 6 shifts per month as needed, including weekends
- Weekend rotations as needed (2–3 per month)
