Responsibilities
- Perform medical necessity determinations and utilization reviews to support appropriate patient care.
- Lead discharge planning and care coordination; identify and communicate care plan changes; refer to community resources; manage length of stay; ensure timely transition planning and discharge documentation.
- Conduct concurrent/continued stay reviews, DRG knowledge, and handling of appeals/denials as needed.
- Maintain HIPAA-compliant, timely documentation, including initial screenings and discharge plans; ensure accurate charting for each patient encounter.
- Collaborate with the interdisciplinary team and coordinate with community resources and DME ordering as appropriate; support adult, pediatric, and geriatric patient populations.
- Demonstrate proficiency with Epic charting and Scubs; ensure consistent, timely entries.

Requirements
- Active RN license in state or compact license required at time of submittal.
- BLS certification.
- Years of experience: 2 years preferred; previous charge experience preferred.
- Education: Registered Nurse (RN) degree from an accredited program; licensure in good standing.
- Charting systems: Epic experience required; familiarity with Scrubs.
- Ability to work effectively in a fast-paced, CMS-focused care setting.
- Onboarding requirements: core items/labs must be completed within 48 hours of contract signing; orientation provided onsite.
