Responsibilities
- Plan, organize and direct home care services to meet patient needs in their homes
- Manage a patient caseload in an effective and efficient manner
- Provide patient care independently, while collaborating with other core disciplines to meet the patient’s plan or care goals
- Function as a resource to patients, family members, and other care team members
- Document care activities and communicate with the care team to support goals and outcomes
- Coordinate with multidisciplinary colleagues to ensure alignment with the patient’s plan of care

Requirements
- Active Registered Nurse license in California
- Experience in case management or home health settings preferred
- Ability to manage a patient caseload and coordinate care across disciplines
- Strong communication skills with patients, families and the care team
- Ability to work independently in a home care environment and adhere to care plans
