Overview
Physical Therapy Assistant (LTAC) in Reading, PA. This travel assignment begins 08/31/2026 for 13 weeks. Expect a weekly pay range of $1,584 to $1,638 with guaranteed hours of 35 per week. Shift is 5x8 days, 08:00 to 16:30. Setting: long-term care / skilled nursing environment.

Responsibilities
- Provide skilled physical therapy services to LTAC residents in collaboration with supervising medical staff.
- Assess mobility, range of motion, strength, balance, and functional limitations; contribute to the development and modification of the individualized plan of care.
- Implement prescribed interventions, including therapeutic exercises, gait training, transfers, and balance activities; monitor patient response and adjust treatment as needed.
- Document treatment sessions accurately and timely; update progress notes and assist with discharge planning and goals.
- Communicate effectively with the rehab team, nursing staff, and patients/families to ensure coordinated care and safety.
- Maintain compliance with regulatory standards, documentation requirements, and facility policies.

Requirements
- Ability to perform PTA duties in a long-term care setting
- Strong communication and teamwork skills
- Ability to follow a plan of care and document patient progress clearly
- Flexibility to meet patient needs and adapt to changing schedules
- Availability to work the stated shift with guaranteed hours
