Physical Therapy Assistant travel assignment in Reading, Pennsylvania. Start date June 1, 2026 for a 13-week contract in a skilled nursing setting. Responsibilities
- Assist the licensed physical therapist in implementing treatment plans for residents requiring rehabilitative care.
- Prepare treatment spaces and equipment, and help patients with setup for therapy sessions.
- Monitor patient responses during therapy, adjust activities within prescribed limits, and report progress to the supervising PT.
- Document treatment sessions accurately in patient records and communicate changes to the care team.
- Support safety and infection control practices; assist with transfers and mobility as directed.
- Collaborate with therapists, nurses, and other staff to support goals in a multidisciplinary care plan.

Requirements
- Ability to work under supervision of a licensed physical therapist and implement prescribed treatment plans.
- Effective communication with patients, families, and care team members.
- Ability to document sessions clearly and timely in patient records.
- Availability to work the stated shift, with the guaranteed hours per week.
- Willingness to work in a skilled nursing setting and participate in team-based care.
