Physical Therapist needed for a Skilled Nursing Facility in Nashua, New Hampshire, United States. Start date 08/31/2026 for a 13-week travel assignment. Weekly pay range $2,463–$2,597 per week with guaranteed 40 hours. Shift is 5x8 days, 08:00–16:30. Setting: long-term care / skilled nursing facility. Responsibilities
- Conduct initial and ongoing PT evaluations to determine resident needs and goals.
- Develop and implement individualized therapy programs to improve mobility, strength, balance, and functional independence.
- Reassess progress, adjust treatment plans, and set measurable outcomes in collaboration with the interdisciplinary team.
- Document evaluations, treatment plans, progress notes, and outcomes in accordance with regulatory and facility standards.
- Provide patient and caregiver education, safety training, and fall-prevention strategies.
- Collaborate with nursing, medical staff, and other therapy disciplines to coordinate comprehensive resident care.

Requirements
- Active Physical Therapist license in New Hampshire or eligible for licensure in NH
- Ability to practice in a skilled nursing facility setting
- Strong documentation and communication skills; comfortable working with a multidisciplinary team
- Available for a 13-week travel assignment with a 40-hour work week
