Responsibilities
- Conduct patient evaluations, develop PT treatment plans, and set measurable rehabilitation goals for residents.
- Deliver evidence-based physical therapy interventions, including therapeutic exercise, gait training, balance activities, and appropriate modalities.
- Monitor progress, adjust treatment plans as needed, and document outcomes in the medical record per regulatory and facility requirements.
- Collaborate with an interdisciplinary team to coordinate care, assist with discharge planning, and educate family members and caregivers on home exercise programs and safety strategies.
- Ensure patient and staff safety, comply with infection control standards, and maintain timely, accurate documentation.

Requirements
- Active Physical Therapist license in Maryland or eligibility for licensure
- Degree from an accredited physical therapy program (DPT or equivalent)
- Ability to work 38 guaranteed hours per week in a day-shift schedule
- Prior experience in a skilled nursing facility or geriatric rehabilitation setting preferred
- Strong documentation, communication, and collaboration skills
- Ability to work effectively in a long-term care environment and meet state regulatory requirements
