Overview
Physical Therapist needed for a travel assignment in a Skilled Nursing Facility setting located in Dallastown, PA, US. Start date 11/01/2026 for 12 contract weeks. Guaranteed 35 hours per week with a 5x8 day shift (08:00–16:30). Weekly pay ranges from $2,418 to $2,492 per week.

Responsibilities
- Conduct PT evaluations and develop individualized treatment plans for adult residents with mobility, balance, strength, and gait deficits.
- Implement evidence-based physical therapy interventions, monitor progress, and adjust plans as needed.
- Document treatment sessions, progress notes, and outcomes in the resident chart; communicate findings to the interdisciplinary team.
- Educate residents, families, and care staff on safe mobility techniques, transfer training, and home exercise programs.
- Collaborate with nursing, OT, speech-language pathology, and other team members to optimize rehabilitation outcomes and discharge planning.
- Ensure therapy services are delivered in a safe, compliant manner and maintain clean, organized treatment areas.

Requirements
- Ability to work in a Skilled Nursing Facility setting and participate in an interdisciplinary care model.
- Strong clinical skills in adult rehabilitation, mobility, and functional training.
- Effective communication and documentation abilities; reliable team collaboration.
- Flexibility to meet the needs of a 12-week travel assignment with guaranteed hours.
- Commitment to resident safety, regulatory compliance, and professional standards.
