Physical Therapist needed for a travel assignment in a skilled nursing facility setting in Albuquerque, New Mexico, United States. Start date: 12/01/2026 for a 13-week assignment with guaranteed 35 hours per week. Weekly pay ranges from $2,447 to $2,601 per week, with a daytime shift on a 5x8 schedule (08:00–16:30). This travel position provides hands-on rehab for residents in a SNF setting and requires collaboration with the interdisciplinary team.

Responsibilities
- Perform initial evaluations and develop individualized treatment plans for SNF residents.
- Administer PT interventions focused on mobility, gait, balance, strength, range of motion, and functional recovery.
- Reassess residents regularly, adjust treatment plans based on progress and goals, and document outcomes.
- Maintain accurate, timely documentation in required medical records and communicate changes to the care team.
- Collaborate with nursing, occupational therapy, speech therapy, and medical staff to coordinate comprehensive care.
- Ensure patient safety, adhere to infection control and regulatory standards, and educate patients and families as appropriate.

Requirements
- Active Physical Therapist license in New Mexico or eligibility for NM licensure
- Doctor of Physical Therapy (DPT) degree or equivalent
- Ability to work in a skilled nursing facility setting and meet the assignment’s schedule
- Strong written and verbal communication skills and ability to collaborate within an interdisciplinary team
- Availability for a 13-week travel assignment with the stated shift hours
- US work authorization
