Responsibilities
- Provide comprehensive physical therapy evaluation and treatment planning for residents in a skilled nursing facility setting.
- Implement individualized treatment programs, including therapeutic exercises, gait training, functional mobility, and restorative modalities.
- Reassess progress, document outcomes in the care plan, and adjust goals as needed in collaboration with the interdisciplinary team.
- Educate residents and families on home exercise programs, safety, and discharge planning.
- Communicate effectively with nurses, physicians, and therapy colleagues to optimize resident outcomes and ensure regulatory compliance.

Requirements
- Active Physical Therapist license in the state of New Hampshire
- Doctor of Physical Therapy (DPT) degree or equivalent
- Ability to work in a skilled nursing facility setting and provide geriatric rehabilitation services
- Availability for a 13-week travel assignment starting 12/01/2026
- Strong communication and teamwork skills to collaborate with an interdisciplinary team
- Eligible to work in the United States
