Overview
Physical Therapist – Skilled Nursing Facility in Jersey Shore, Pennsylvania. This travel, temporary assignment starts 07/13/2026 and runs for 13 weeks with guaranteed 35 hours per week. The shift is 8:00 a.m. to 4:30 p.m. The weekly pay range is $2,219 to $2,323. This assignment takes place in a long-term care setting serving residents in a skilled nursing facility.

Responsibilities
- Perform initial physical therapy evaluations and ongoing progress assessments for residents in a long-term care setting.
- Develop and implement individualized treatment plans aimed at improving mobility, strength, balance, gait, and functional independence.
- Deliver evidence-based interventions such as therapeutic exercises, gait training, manual therapy, and neuromuscular re-education as appropriate.
- Monitor and document resident progress, adjust goals as needed, and communicate changes to the interprofessional care team.
- Collaborate with nursing, occupational therapy, speech-language pathology, and social work to coordinate care and discharge planning.
- Educate residents and families on home exercise programs, safety, fall prevention, and device or equipment use as part of ongoing recovery.

Requirements
- Experience in a skilled nursing or geriatric long-term care setting preferred
- Ability to work a full-time 8:00 a.m. – 4:30 p.m. schedule during a 13-week assignment
- Strong documentation, organizational, and communication skills
- Comfortable functioning within a multidisciplinary care team and a regulatory environment
- Willingness to provide patient- and family-centered care for an aging population
- Availability to begin on 07/13/2026 and commit to the contract duration
