Physical Therapist – Skilled Nursing Facility – Pembroke, North Carolina. Travel (temporary assignment) starting 07/13/2026 for 13 weeks. Guaranteed 35 hours per week. Shift: 5x8 days (08:00–16:30). Responsibilities
- Assess residents’ physical abilities, mobility, gait, strength, and range of motion; develop individualized treatment plans.
- Deliver evidence-based physical therapy interventions to improve functional independence and reduce risk of decline.
- Reassess progress regularly; document outcomes and adjust plans to meet short- and long-term goals.
- Collaborate with an interdisciplinary care team to coordinate care, communicate progress, and support discharge planning.
- Provide patient and family education, including home exercise programs and safety strategies for daily activities.
- Maintain accurate, timely documentation in charting systems and ensure compliance with regulatory standards.

Requirements
- Doctor of Physical Therapy (DPT) degree
- Active Physical Therapist license in North Carolina or eligibility for licensure in North Carolina
- CPR/BLS certification (or willingness to obtain)
- At least 1 year of physical therapy experience in a skilled nursing facility or similar setting preferred
- Strong communication skills and ability to work within a multidisciplinary team
- Ability to work the 13-week assignment with guaranteed hours and a 5x8 schedule
