Physical Therapist | Skilled Nursing Facility | Dallastown, Pennsylvania, US. Start date 08/03/2026 for a 13-week travel assignment. Weekly pay range $2,125–$2,274 per week, with guaranteed hours of 35 per week. Shift schedule: 5x8 days, with work hours 08:00–16:30. Setting: long-term care / nursing home environment.

Responsibilities
- Perform comprehensive physical therapy evaluations and reassessments for residents in a SNF setting.
- Develop, implement, and modify individualized PT treatment plans aligned with resident goals and care team input.
- Deliver evidence-based interventions (gait training, therapeutic exercise, balance, mobility, ROM, functional activities) to improve or maintain function.
- Monitor progress, document outcomes, and communicate changes to the interprofessional team; maintain accurate, timely records.
- Educate residents and families on home exercise programs, safety strategies, and discharge planning; provide staff training as needed.
- Ensure resident safety, infection control, and adherence to facility policies and regulatory requirements.

Requirements
- Active physical therapy license (state-specific) or eligible to practice per state requirements
- Ability to work in a skilled nursing facility setting and participate in a travel assignment
- Willingness to fulfill a 13-week assignment and maintain scheduled hours
- Strong communication and documentation skills; ability to collaborate with an interdisciplinary team
- Availability to work the specified shift: 08:00–16:30, 5 days per week
