Overview
Physical Therapist needed for a Skilled Nursing Facility in Siler City, North Carolina. Start date: 08/10/2026. This travel assignment runs for 13 weeks with guaranteed 35 hours per week. Shift: 08:00–16:30, five days per week (5x8). Setting: long-term care / residential care. Compensation: $1,849–$1,915 per week. Responsibilities
- Conduct initial physical therapy evaluations and ongoing re-evaluations for residents in a long-term care setting.
- Develop and implement individualized treatment plans to improve mobility, strength, balance, gait, and functional independence.
- Provide therapeutic interventions (therapeutic exercises, neuromuscular re-education, and appropriate modalities) to address identified deficits.
- Monitor progress, document sessions in the patient record, and adjust goals as residents progress.
- Collaborate with an interdisciplinary team (nursing, occupational therapy, speech-language pathology, social work) to optimize care and discharge planning.
- Educate residents and families on home exercise programs, safety strategies, fall prevention, and activity modification.

Requirements
- Active Physical Therapist license in North Carolina
- Ability to work in a long-term care / residential care setting
- Strong documentation and written/verbal communication skills
- Availability to work the 08:00–16:30 shift, five days per week
- Adaptability to a travel assignment environment and routine
