Overview
Physical Therapist – Skilled Nursing Facility travel assignment in Albuquerque, NM, US. Start date 07/27/2026 for 13 weeks. Weekly pay range $2,264–$2,387 per week with guaranteed hours of 36. Shift is 08:00–16:30, five days per week. This is a travel - temporary assignment through Healthforce Healthcare LLC.

Responsibilities
- Conduct initial PT evaluations and develop individualized plans of care for adults in a skilled nursing setting.
- Provide therapy interventions including therapeutic exercise, gait and balance training, ROM and strength work, and functional mobility tasks.
- Monitor patient progress, document outcomes, and adjust the plan of care as needed in accordance with policy.
- Collaborate with interdisciplinary team members (nursing, OT, SLP, physicians) to optimize patient outcomes and discharge planning.
- Ensure patient and staff safety during treatment sessions and maintain infection control standards.
- Educate patients and families on home exercise programs and strategies to promote independence.

Requirements
- Doctor of Physical Therapy (DPT) degree or equivalent
- Active Physical Therapist license in the United States with eligibility to practice in New Mexico
- Current CPR/BLS certification
- Preferred: 1+ year of PT experience in geriatric or skilled nursing rehabilitation
- Ability to work in a skilled nursing facility environment and participate in a 13-week travel assignment
- Agencies must have a NM caregivers account to engage with the assignment
