Weekly pay: $2,348–$2,484 per week | Guaranteed hours: 35
This travel assignment offers an increased rate for a LT C nursing setting. You will provide patient-focused physical therapy services in a long-term care environment, working a daytime schedule across an approximate 13-week contract.

Responsibilities
- Assess residents to determine PT needs and establish individualized treatment plans.
- Deliver skilled PT interventions to improve mobility, strength, balance, and functional independence.
- Reassess progress regularly and adjust treatment plans to meet outcomes.
- Document evaluations, progress notes, and discharge summaries in the facility’s health record system.
- Collaborate with an interdisciplinary team (nursing, rehab disciplines, physicians) to coordinate care and support discharge planning.
- Maintain safety, infection control, and patient privacy; educate residents and families about home exercise programs and safety strategies.

Requirements
- Active Physical Therapist license in New Hampshire or eligibility for licensure.
- Doctor of Physical Therapy (DPT) degree.
- CPR/BLS certification.
- Experience in a skilled nursing or geriatric rehabilitation setting preferred.
- Ability to work in a long-term care environment and participate in a travel assignment.
- Strong communication skills and ability to work within an interdisciplinary team.
