Overview
Physical Therapist – Outpatient, travel assignment in Lebanon, Oregon, starting 09/28/2026 for 13 weeks. Weekly pay range: $2,365–$2,423 per week. Guaranteed hours: 40 hours per week. Shift: Days, 4x10-hour shifts (0700–1700) with no weekend coverage. Work setting: hospital-based outpatient rehabilitation department.

Responsibilities
- Conduct initial evaluations and ongoing treatment for outpatient patients with injuries or conditions affecting mobility and functional independence.
- Develop, implement, and adjust individualized rehabilitation plans; monitor progress and modify interventions as needed.
- Provide therapy interventions such as therapeutic exercise, gait training, manual therapy, and modalities within scope and department guidelines.
- Document patient assessments, treatment plans, goals, and progress in the medical record; communicate with the care team and document patient education provided.
- Educate patients and families on home exercise programs, safety strategies, and functional adaptations to support continued recovery.
- Collaborate with a multidisciplinary team to coordinate care and ensure seamless patient transitions across services.

Requirements
- Current unencumbered Oregon Physical Therapy license
- Healthcare Provider Level BLS certification
- Ability to travel to multiple points of care
- Strong clinical documentation and communication skills
- Ability to work in a hospital-based outpatient rehabilitation environment
- Outpatient rehabilitation experience preferred
