Overview
Physical Therapist – SNF travel assignment in Santa Fe, New Mexico, United States. Start date June 1, 2026 for approximately 13 weeks. Weekly pay range $2,142–$2,193 per week. Guaranteed hours 35 per week. Shift from 8:00 a.m. to 4:30 p.m., Monday through Friday. Setting: skilled nursing facility. This is a travel - temporary assignment managed by Healthforce Healthcare LLC.

Responsibilities
- Evaluate residents to determine PT needs and develop individualized treatment plans within scope of practice.
- Implement therapeutic interventions including therapeutic exercise, gait training, balance, strength, and functional activities.
- Monitor progress, document treatment sessions, and adjust plans based on patient response and goals.
- Administer modalities and assistive devices as appropriate, ensuring patient safety and comfort.
- Collaborate with an interdisciplinary team to support discharge planning and optimize continuum of care.
- Provide patient and family education, and establish or update home exercise programs as part of transition planning.

Requirements
- Active Physical Therapist license in New Mexico or eligibility for licensure in New Mexico.
- Master’s or Doctoral degree in Physical Therapy.
- Experience in post-acute rehab or skilled nursing facility (SNF) settings preferred.
- Basic Life Support (BLS) certification preferred.
- Ability to work effectively in a skilled nursing facility environment and participate in a travel assignment.
- Strong communication and collaborative skills with an interprofessional team.
