Guaranteed hours: 35 per week. Shift: 5x8 Days (08:00–16:30). Setting: Skilled Nursing Facility in a travel, temporary assignment.

Responsibilities
- Conduct physical therapy evaluations and develop individualized treatment plans for residents in a skilled nursing setting.
- Implement evidence-based therapies to improve mobility, strength, balance, gait, and functional independence.
- Administer therapeutic exercises, manual therapy, gait training, and assistive device training as clinically indicated.
- Monitor patient progress, document outcomes, and adjust treatment plans in collaboration with the interdisciplinary team.
- Educate residents and families on home exercise programs, safety, fall prevention, and activity modification.
- Maintain accurate documentation in medical records and ensure timely communication with supervising clinicians and the care team.

Requirements
- Active physical therapy license in New Hampshire or license eligibility.
- Doctor of Physical Therapy (DPT) degree from an accredited program.
- Ability to commit to a 13-week travel assignment with guaranteed 35 hours per week.
- Experience in a skilled nursing facility or geriatric rehabilitation setting preferred.
- Strong communication and collaboration skills; able to work within an interdisciplinary team.
- Eligibility to work in the United States without sponsorship (as required for this assignment).
