Responsibilities
- Conduct comprehensive PT evaluations and develop individualized plans of care to address mobility, gait, strength, balance, and functional goals.
- Deliver evidence-based physical therapy interventions and therapeutic activities to improve residents’ independence and safety.
- Reassess progress regularly, update treatment plans, and document outcomes in the resident’s record.
- Collaborate with an interdisciplinary team to optimize discharge planning, goals, and family education.
- Ensure compliance with regulatory and facility policies; participate in quality improvement and performance improvement activities as needed.

Requirements
- Active Physical Therapist license in the United States (state licensure appropriate for assignment; NC licensure preferred or eligible).
- Doctor of Physical Therapy (DPT) degree or equivalent required.
- Experience in skilled nursing facilities or geriatric PT is preferred.
- CPR certification is preferred.
- Strong communication skills and ability to document clearly; ability to work effectively within an interdisciplinary team.
- Availability for a 14-week assignment with the stated weekly hours and shift.
