Responsibilities
- Perform PT evaluations and develop individualized treatment plans for residents in a skilled nursing setting.
- Implement therapeutic interventions to improve mobility, strength, balance, gait, and functional independence.
- Monitor patient progress, adjust plans as needed, and document assessments, treatment plans, and progress notes.
- Collaborate with interdisciplinary team including nursing, therapy, and medical staff; communicate patient status and discharge planning.
- Ensure patient and staff safety during treatments; apply modalities and assistive devices as ordered.
- Prepare discharge plans and provide patient and caregiver education to support continued progress.

Requirements
- Doctor of Physical Therapy (DPT) degree
- Active or eligible California physical therapy license
- Experience in or willingness to work in a skilled nursing/long-term care setting
- Ability to commit to a 13-week travel assignment with a minimum of 30 hours per week
- Strong documentation and communication skills
- Capacity to work the requested Sunday–Thursday or Tuesday–Saturday schedule
