Responsibilities
- Evaluate patients and develop individualized treatment plans for post-acute rehabilitation
- Implement evidence-based physical therapy interventions to improve mobility, strength, balance, and functional independence
- Monitor patient progress, adjust treatment plans, and document outcomes in the medical record
- Contribute to goal setting and discharge planning in collaboration with the interdisciplinary team
- Provide patient and family education on home exercise programs, safety, and equipment use
- Ensure all care complies with facility policies, safety guidelines, and regulatory standards

Requirements
- Active physical therapy license to practice (California) or as required by state regulations
- Doctor of Physical Therapy (DPT) degree or equivalent
- Ability to work in a skilled nursing facility setting
- Availability for a 13-week travel assignment starting 06/15/2026 with a Monday–Friday 40-hour schedule
- Strong clinical documentation, communication, and collaboration skills
