Overview
Physical Therapist, Home Health — Belle Fourche, South Dakota, United States. Start date 06/22/2026. 13-week travel assignment in a long-term care setting with a guaranteed 40 hours per week. Days shift. Weekly pay range: $2,265–$2,487 per week. This temporary assignment requires working in a nursing home environment while delivering PT services across resident care plans.

Responsibilities
- Perform comprehensive physical therapy evaluations and establish individualized treatment plans for residents.
- Implement PT interventions including therapeutic exercises, gait and balance training, range-of-motion work, transfer training, and functional mobility.
- Monitor and document patient progress, adjust treatment plans, and communicate outcomes to the care team.
- Provide patient and family education on exercises, safety, fall prevention, and home exercise programs; ensure continuity of care with interdisciplinary staff.
- Maintain accurate, timely documentation in compliance with regulatory requirements and facility policies.
- Ensure safety and infection control during all interventions and prioritize resident dignity and quality of life.

Requirements
- Active Physical Therapist license in good standing in the United States.
- Ability to work in a long-term care setting and collaborate with an interdisciplinary team.
- Availability to start on 06/22/2026 and commit to a 13-week travel assignment.
- Strong documentation and communication skills; ability to develop and update care plans and progress notes.
- Adaptability to a days shift schedule and to working within a nursing home environment.
