Physical Therapist, Home Health, in Glendive, MT is needed for a travel assignment starting May 18, 2026. Responsibilities
- Deliver physical therapy services to residents per plan of care, including evaluation, diagnosis, and treatment planning.
- Implement therapeutic interventions to improve mobility, balance, strength, and functional independence.
- Reassess residents regularly, adjust treatment plans, and document progress toward rehabilitation goals.
- Communicate and collaborate with an interdisciplinary team to coordinate care, discharge planning, and ongoing needs.
- Maintain accurate, timely documentation and ensure compliance with regulatory, payer, and facility policies, including privacy and safety standards.
- Promote resident safety, infection control, and appropriate use of assistive devices and modalities in daily care.

Requirements
- Ability to work a Day shift in a long-term care / skilled nursing setting.
- Demonstrated capability to evaluate and treat adults and elderly residents within a rehabilitation framework.
- Strong communication and documentation skills, with attention to care coordination and regulatory compliance.
- Adaptability to a fast-paced environment and collaboration with nursing staff and therapists.
- Willingness to adhere to facility policies and participate in standard professional routines and care planning.
- Eligibility to participate in a travel assignment and meet any credentialing requirements set by the hiring organization.
