Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical, social and mental functions, while meeting department objectives.
- Serve as co-leader in an interdisciplinary approach to patient groups.
- Monitor patient progress and adjust treatment plans to achieve goals; document outcomes.
- Educate patients and caregivers on exercises, safety, and home exercise programs.
- Collaborate with an interdisciplinary care team to coordinate care and support discharge planning.
- Ensure safe application of therapy in patients’ home environments and adapt activities as needed.

Requirements
- Doctor of Physical Therapy degree (or equivalent) and eligibility to practice in California
- Ability to work in a home health setting on a 13-week travel assignment
- Strong communication and teamwork skills for interdisciplinary collaboration
- Proficiency in documentation, outcomes measurement, and regulatory compliance
- Availability to begin on 05/18/2026 and commit to the 13-week duration
