Overview
Physical Therapist, General – Sonora, California
Travel assignment starting 09/21/2026 for 13 weeks with guaranteed 36.0 hours per week. Weekly pay range of $2,507 to $2,606, with 5x8 day shifts (typical 8:00 AM–4:30 PM). Setting: Short Term Acute Care Hospital. This is a travel - temporary assignment through Healthforce Healthcare LLC.

Responsibilities
- Plan and conduct segments of physical therapy treatment programs to restore and improve physical, social, and mental function.
- Serve as co-leader in an interdisciplinary patient care approach.
- Complete comprehensive initial evaluations and ongoing assessments to inform individualized treatment plans.
- Provide therapy interventions using evidence-based protocols and document progress toward goals.
- Develop discharge plans and communicate plans to appropriate team members and patients/families.
- Complete accurate clinical documentation within established deadlines; supervise assigned assistants as needed.

Requirements
- Doctor of Physical Therapy (DPT) degree: Required; only if obtained license on or after January 2016; or foreign equivalent.
- Physical Therapy licensure in all applicable states of practice: Required
- CPR certification or Basic Life Support (BLS, HS-BLS, or RQIBLS) from an approved vendor per policy: Required
- Valid Driver’s License and must be at least 21 years of age: Required
- At least two years of recent experience in the field
- Current BLS certification
- EPIC electronic medical record experience
- Current nursing license per the state of assignment
